Add a kamal doctor that checks ssh/docker reachability, registry + ghcr auth, proxy version vs MINIMUM_VERSION, ports 80/443 free, cert expiry, and DNS→host resolution before a deploy.

Pattern: zoolutions/pgbus#212 doctor CLI.

Verify: kamal doctor reports pass/fail per check against a real target.
