kamal-proxy deploy supports --health-check-port/--health-check-host (merged upstream in basecamp/kamal-proxy#152) but the gem never plumbs them into deploy.yml.

Demand: basecamp/kamal#1234 (11 comments), basecamp/kamal#1842.

Anchor: flag map in lib/kamal/configuration/proxy.rb:91-123.

Verify: healthcheck.port/healthcheck.host in deploy.yml emit the flags; test/commands/proxy_test.rb assertions.
