Operational Readiness & Observability Assessment

This PR adds operational runbook documentation, observability stack assessment, and GitHub incident report templates for tuna-os/docs as permitted under operations policy.

• .github/ISSUE_TEMPLATE/incident_report.md & postmortem.md: Operational incident report and postmortem templates.
