docs: preserve plain output without substitute decoration - #4354

Define the plain quoted-output form and make the examples match it. Put output design guidance before implementation details so partial reads reach the contract. Keep an extraction-decision replay that distinguishes the old and corrected guidance.

One wording nit inline: the gutter section has no examples beneath it. The plain-form gutter examples sit both earlier in the skill (auto-staging, adding to user config) and later (command failures, template blocks).

Plain output preserves meaning through wording, spacing, and existing semantic symbols. Update the output skill’s graceful-degradation rule, define the gutter’s plain form as a two-space indent, and correct its examples in their existing sections.
Validation: the complete repaired skill rejected the literal-gutter proposal in both trials, while the baseline promoted it in both. Three related controls passed twice each. The full local pre-merge gate passed: 5,048 tests, doctests, rustdoc, and pre-commit.
