Figure out how to handle the jurisdiction stuff #13

Description

@BryceStevenWilley

The jurisdiction switcher doesn't work great at the moment (lets you switch in the middle of making a case, everything breaks when it does), but that's fine; it's more of a dev tool than a final polished feature.

What does the final polished feature look like though? An additional path on the URL (/massachusetts/expert_form vs /illinois/expert_form?) or different domains / sites entirely? This is more of an open question; with docassemble interviews, whoever hosts the interview hosts the domain, so it's all different sites, but this is the first time one group (Suffolk), will be maintaining multistate user-facing things.

    My recommendation would be to use paths for now, because making things multi-domain later is much simpler than merging domains later.

    This is now done (from #28 I believe, though probably had some clean ups in #29 and #32).
