Link the encounter audit's patient at the OpenMRS 2.x dashboard - #45

The patient's name in the Encounter Summary section pointed at the O3
patient chart. Point it at the legacy clinician dashboard instead, which
is where an auditor goes to read the rest of the record:
/openmrs/pihcore/router/programDashboard.page?patientId=<uuid>
Renames the config key from patientChartUrl to patientDashboardUrl, since
it no longer names a chart. The default uses ${openmrsBase} rather than a
literal /openmrs so that it resolves correctly on deployments served from
another context path. The target is outside the SPA, so ConfigurableLink
falls back to a full page load.
