Although there weren't any comments for this to begin with, it would make review/maintenance a bit easier if there was a bit of explanation for each group of rules. I will try to add some when I review, but I'm on vacation for the next week, so if you're able to get to that before I can, that would help.

I added some explanations. They're generally helpful. Frankly, I tend to research these kinds of things for a while and then completely forget what they are.

It should be, IMHO. We're still seeing .php in the URL
