Correct vendoring docs for the tools-lsp exception
The all-vendored claim in AGENTS.md and the VENDORING.md intro left out that @intx/tools-lsp stays on published npm, and the workflow-host sync instruction would have re-copied eleven upstream adapter files that the vendored tree intentionally omits.
