Additions to coding standard

During reviews comments are made about the formatting of the code.
Those comments that are not covered by the current coding standard should be formally introduced.

This issue is to formally add the following standards based on review comments:

• Vertical spacer before loops.
• Vertical spacer after in-code comment blocks.

Now is the time to comment if any of these proposed additions are not welcome.

We might want to add:

• Vertical spacer between variable declarations and code itself.
• Use 120-character lines (we started with 80-character lines, but I find this a bit restrictive).

As a general rule, I like my code to be "airy". It makes it easier/faster for me to spot things.
